OH MY GOD. That piano. I had forgotten about it. GOSH, King Boo really had some of the creppiest things in his manor.

Generic response, but...the Moon from Majora's Mask.

The Golden Monkey in the Golden Compass game. Scary in the movie, scarier in the game.

Also, I don't know if these really scare me, but the darkened rooms in Goof Troop make me really nervous, because I can never tell where the enemies are and I waste a bunch of lives

And last but not least, the VENUS FLY TRAPS IN LUNATEA'S VEIL. I DON'T KNOW WHY BUT THEY'RE REALLY SCARY

That piano (i played it in my DS), my reaction was, "W-whoa!"

When I was little, I was pretty much traumatized by the movie "Jaws". So for a while, I was scared of water levels in general, because I'd always assume that a shark would chase after me...

This.
To be honest, the music on this map freaked me out more than John Romero's head back in the day. The music actually sounds the same when you play it backwards! :O

Game Over screens. Notably the ones from SNES Starfox, Smash Bros. 64, and Banjo-Kazooie. These ones scared the ever-loving cr*p outta me growing up as a backseat gamer.
I was pretty afraid of game over screens too, for some reason. I think it was Mega Man Legends that had this game over sequence that was a pitch black screen and there was an ominous sound effect leading up to a big red 'Game Over'. Not sure why it made me so afraid as to run away from my room.

Also when a glitch happens that makes the game freeze with that bleeping sound. That scared me so much. I used to completely stop playing certain games just because they froze that way once.

I don't really know if this counts but when I was little I put a .EXE file (That was a game) into the audio player when that still worked thinking it would play the game's soundtrack sort of like a PS1 disk and instead got the pleasant surprise of
